Roman Catholic and that you would like your religion recorded. If you are not recorded as a Roman Catholic,
your name will not be given to the Chaplain for Holy Communion on Sunday morning. If you require any pastoral assistance, please ask the Ward to bleep the Roman Catholic Chaplain.
